As part of our current Brain and Behaviour module, we had a lecture on schizophrenia: the second statistic we received was that an estimated 30% of homeless people in the US have schizophrenia (allowing for, as you said, diagnoses based on it being the closest match). That remained very present in my mind when Sherlock started talking about the drug trial and it became more clear what was going on.
